Life/AD&D and
Voluntary Benefits

Full-time employees will receive 1 x salary to a maximum of $100,000 of Life/AD&D coverage at no cost to you. 

​

In addition to the Basic Life offered through Reliance Standard you have an opportunity to purchase additional life insurance for yourself, spouse and child(ren) to help financially protect your family. You must enroll in the coverage in order to cover your dependents. Evidence of Insurability (EOI) is required if enrolling over the Guarantee Issue (GI) amount or if you are a late entrant (someone who waived voluntary life/AD&D coverage during their initial new hire eligibility period).

​During this annual Open Enrollment period, you may increase your existing Voluntary Life/AD&D coverage by $50,000 (in $10,000 increments) for Employees, and by $10,000 (in increments of $5,000) for Spouses, without EOI (Evidence of Insurability).

Voluntary Life Insurance Benefits 

Employee 

Increments of $10,000 up to a maximum of $500,000 (not to exceed 5 times salary) 

Guarantee issue amount $150,000

Spouse

Increments of $5,000 to a maximum of $500,000 (not to exceed 50% of employee amount) 

Spouse Guarantee issue amount $25,000

Child(ren)

Increments of $2,000 to a maximum of $10,000

(Ages birth to 21 years, 26 if full time student) 

 

Rate is $.29/$.042 per $1,000 of life insurance/AD&D coverage

(One rate regardless of the number of children covered). 

Accident, Critical Illness, & Hospital Indemnity

Accident Insurance 

Accident Insurance is designed to help covered employees with additional financial support to meet the expenses and extra bills that can follow an accidental injury, whether minor or catastrophic, such as co-pays, transportation, and out-of-pocket fees. Lump sum benefits are paid directly to the employee based on the amount of coverage listed in the schedule of benefits. Hospital Indemnity Insurance pays a fixed, lump-sum benefit to help cover expenses resulting from a covered hospitalization and is paid directly to you

Critical Illness Insurance 

Critical Illness insurance helps offset the financial effects of a catastrophic illness by paying a lump sum benefit when an employee or their family members are diagnosed with a covered illness. The benefit is based on the amount of coverage in force and the specific diagnosed illness.

“During this annual Open Enrollment period, you may elect up to $30,000 of Critical Illness coverage without EOI (Evidence of Insurability)”.

Hospital Indemnity Insurance pays a fixed, lump-sum benefit to help cover expenses resulting from a covered hospitalization and is paid directly to you.
