
Eligibility
BENEFIT ELIGIBILITY

An employee working 30 or more hours per week is considered a full-time employee; once full-time eligibility is established, benefits will begin on:
First of the month following 30 days of employment.
Family members including spouse or domestic partner and dependent child(ren) are also eligible for coverage.

ELIGIBLE DEPENDENTS INCLUDE:

Legally married spouse

Domestic partner
-
Child up to the age of 26 including;
− Natural or adopted child
− Step child
− Child that you are legal guardian
− Child that has been placed with you for adoption
Once you have made your elections, you will not be able to make any changes until the next annual open enrollment period, unless you experience a Qualifying Event, as defined by the IRS.

QUALIFYING EVENT

If you experience a Qualifying Event, you can make a change to your benefit that is consistent with the status change.

QUALIFYING EVENTS INCLUDE:

Marriage

Divorce / Legal separation
-
Birth / Adoption / Legal custody of a child

Death of a spouse or dependent child

Employee or dependents gain/loss of other coverage
-
A change in work status that causes you to gain or lose eligibility.

When a qualifying event occurs during the plan year please remember you have 31 days to make an election.
